      Abstract
      MUHAMMAD RIFQI KEIZA SYAM. Survei Kematangan Kualitas dan Budaya Keamanan Pangan Berdasarkan FSSC 22000 Versi 6.0 di PT XYZ. Dibimbing oleh SLAMET BUDIJANTO dan GABRIELLA MASTA ELIZABETH MANALU. Penerapan Quality and Food Safety Culture (QFSC) menjadi faktor penting dalam mendukung efektivitas Sistem Manajemen Keamanan Pangan (SMKP), khususnya pada industri pangan yang menerapkan standar internasional seperti Food Safety System Certification (FSSC) 22000 versi 6.0. Penelitian ini bertujuan untuk mengukur tingkat penerapan QFSC melalui nilai Food Safety Culture Maturity Index (FSCMI) dan maturity level. Pengukuran dilakukan menggunakan survei melalui kuesioner berbasis skala Likert 1 – 5 yang telah diuji validitas dan reliabilitasnya berdasarkan empat elemen, yaitu komunikasi, pelatihan, umpan balik karyawan, dan pengukuran kinerja. Hasil penelitian ini menunjukkan bahwa nilai FSCMI perusahaan sebesar 4,41 yang berada di level 4 (predict), dengan capaian masing-masing elemen yaitu komunikasi (4,43), pelatihan (4,33), umpan balik karyawan (4,47), dan pengukuran kinerja sebesar (4,40). Hasil ini menggambarkan penerapan QFSC telah berjalan secara sistematis dan konsisten serta mampu mengantisipasi potensi risiko melalui proses evaluasi. Selain itu, hasil uji korelasi Spearman menunjukkan bahwa posisi kerja dan masa kerja memiliki hubungan yang signifikan dan berkorelasi positif terhadap implementasi budaya mutu dan keamanan pangan dengan nilai koefisien korelasi masing-masing sebesar 0,619 dan 0,672 (p < 0,05).       MUHAMMAD RIFQI KEIZA SYAM. Survey on Quality and Food Safety Culture Maturity Based on FSSC 22000 Version 6.0 at PT XYZ. Supervised by SLAMET BUDIJANTO and GABRIELLA MASTA ELIZABETH MANALU. The implementation of the Quality and Food Safety Culture (QFSC) is a key factor in supporting the effectiveness of the Food Safety Management System (FSMS), particularly in the food industry that adopts international standards such as Food Safety System Certification (FSSC) 22000 Version 6.0. This study aims to measure the level of QFSC implementation through the Food Safety Culture Maturity Index (FSCMI) score and maturity level. The measurement was conducted using a survey via a 1–5 Likert-scale questionnaire that had been tested for validity and reliability based on four elements: communication, training, employee feedback, and performance measurement. The results of this study indicate that the company’s FSCMI score is 4.41, placing it at Level 4 (predict), with scores for each element as follows: communication (4.43), training (4.33), employee feedback (4.47), and performance measurement (4.40). These results indicate that the implementation of QFSC has been carried out systematically and consistently and is capable of anticipating potential risks through the evaluation process. Additionally, the results of the Spearman correlation test indicate that job position and tenure have a significant and positive correlation with the implementation of quality and food safety culture, with correlation coefficients of 0.619 and 0.672, respectively (p < 0.05).
