This study aimed to analyze the causes of defects and evaluate the effectiveness of improvement actions in the biscuit packaging process using the Six Sigma DMAIC (Define, Measure, Analyze, Improve, and Control) approach at PT XYZ. Data were collected through production observations, defect records, and interviews with production operators and the Quality Control department. The results showed that the dominant defect was printing misalignment, accounting for 3,687 units or 63.37% of the total defects. Fishbone and 5 Whys analyses identified the material factor as the main cause of the defect. Improvement was implemented by replacing the packaging film with material having more suitable surface characteristics and consistent thickness. After implementation, the number of defects decreased from 5,818 to 2,520 units, the defect rate decreased from 1.85% to 0.80%, the DPMO value decreased from 6,181 to 2,677, and the sigma level increased from 4.00 to 4.29. These findings indicate that the Six Sigma DMAIC approach effectively improved the packaging process quality and reduced production losses at PT XYZ.
