Air pollution remains one of the major environmental challenges worldwide, including in Indonesia. The transportation sector is a major contributor to air quality deterioration through vehicle exhaust emissions containing pollutants such as carbon monoxide (CO), hydrocarbons (HC), nitrogen oxides (NOx), sulfur dioxide (SO2), and particulate matter. The continuous increase in the number of motor vehicles has resulted in higher fuel consumption and consequently greater atmospheric emissions. This condition poses significant risks to public health, particularly respiratory diseases and other chronic illnesses associated with long-term exposure to air pollutants. Various strategies have been implemented to mitigate air pollution, including the enforcement of stricter emission standards and the development of vehicle emission control technologies. One promising approach is the application of fuel additives to improve combustion quality, thereby promoting more complete combustion, enhancing fuel efficiency, and reducing exhaust emissions. The objectives of this study were to analyze exhaust emissions resulting from fuel combustion with the addition of additives, determine the combustion efficiency of Pertalite fuel with additive application in motorcycles and generator sets, determine Pertalite fuel consumption associated with emission reductions and improvements in combustion efficiency, and assess the correlation between emission reductions and health effects. The study was conducted at the IPB Integrated Laboratory using two emission sources, namely a 125 cc automatic motorcycle representing a mobile emission source and a 10 kVA gasoline-powered generator representing a stationary emission source. Three fuel additives, namely Bardahl, Lupromax, and IPB-eMReTe, were evaluated at concentrations of 0.25%, 0.50%, 0.75%, and 1.00% (v/v) blended with Pertalite gasoline. Engine performance was tested under two operating conditions: idle (800–1,500 rpm) and loaded (>3,000 rpm). Exhaust emissions of carbon dioxide (CO2), carbon monoxide (CO), hydrocarbons (HC), nitrogen oxides (NOx), and sulfur dioxide (SO2) were measured. CO, CO2, and HC concentrations were analyzed using an Automotive Gas Analyzer, whereas NOx and SO2 were measured using a Flue Gas Analyzer following the Indonesian National Standard (SNI 09-7118.3-2005). Each treatment was conducted in triplicate. The results demonstrated that fuel additives generally improved combustion performance in both the motorcycle and the generator. Improved combustion was indicated by increased CO2 concentrations, reflecting more complete combustion, accompanied by decreased CO concentrations, which represent incomplete combustion products. For the motorcycle, combustion efficiency increased from approximately 67–73% under loaded conditions without additives to more than 90% after additive application. IPB-eMReTe exhibited the highest improvement in combustion efficiency under loaded conditions with a coefficient of determination (R²) of 0.847, followed by Lupromax (R² = 0.796) and Bardahl (R² = 0.673). Similar improvements were observed in the generator under both idle and loaded conditions, with combustion efficiencies exceeding 90% at additive concentrations of 0.75–1.00%. These findings indicate that fuel additives enhanced the homogeneity of the air–fuel mixture, thereby promoting a more efficient oxidation process during combustion. Fuel additives also significantly reduced exhaust emissions. In the motorcycle, IPB-eMReTe achieved the highest reduction in carbon monoxide emissions (88.33%), followed by Lupromax (84.99%) and Bardahl (76.01%). For hydrocarbon emissions, Lupromax produced the greatest reduction (92.03%), followed by Bardahl (87.47%) and IPB-eMReTe (86.91%). All additives also reduced NOx emissions from both the motorcycle and the generator, although the reductions were less pronounced than those observed for CO and HC because NOx formation is primarily governed by combustion temperature. Duncan's Multiple Range Test confirmed that additive application produced statistically significant differences compared with the control treatment for most emission parameters. These results demonstrate that fuel additives improve combustion quality while simultaneously suppressing the formation of incomplete combustion pollutants. The reduction in pollutant concentrations directly contributes to a decrease in public health risks. An Environmental Health Risk Assessment (EHRA) was conducted using different exposure durations (1, 3, and 8 hours). Under conditions without additive application, the Risk Quotient (RQ) values were 0.01, 0.04, and 0.12 for CO; 0.49, 1.48, and 3.95 for HC; and 0.003, 0.01, and 0.02 for NO2, respectively. HC RQ values exceeding 1 indicate a potential non-carcinogenic health risk associated with hydrocarbon exposure. Following the simulation of 1% IPB-eMReTe additive application, the RQ values decreased to 0.002, 0.005, and 0.01 for CO; 0.06, 0.19, and 0.52 for HC; and 0.002, 0.005, and 0.01 for NO2, respectively. These reductions indicate that the application of the additive can substantially reduce the level of pollutant exposure. The reduction in emissions subsequently affects the estimated health impacts and their economic valuation. Lower pollutant concentrations result in a decrease in the estimated number of adverse health cases among the exposed population, thereby reducing the total economic losses attributable to air pollution. The Total Economic Cost (TEC) analysis showed that health-related costs decreased from approximately IDR 221,468,788 under the condition without additive application to IDR 192,504,576 following the application of 1% IPB-eMReTe, corresponding to cost savings of IDR 28,964,212 (13.08%). These findings demonstrate that the application of the additive provides not only technical benefits through improved combustion efficiency and reduced fuel consumption, but also environmental and public health benefits, as well as economic benefits through reductions in the costs associated with air pollution-related diseases.
