      Abstract
      This final project report discusses the high number of defect flashes and dirty defects in the Cap PPM 03 product observed in the company's defect data from May to October 2023. The author analyzes the causes of defects in the product. The purpose of this research is to reduce the number of defects in the company's products, thereby minimizing production process costs and maximizing profits. In this study, problem analysis is conducted using the Six Sigma method with DMAIC stages and involves seven tools control methods to help identify quality issues, analyze root causes, and find effective solutions. Based on the research results, the lowest sigma value was found to be 3.52, with an average of 3.80. The root cause analysis using a fishbone diagram revealed that the primary cause of defect flashes and dirty defects originated from the machine. The study results then proposed the creation of a standard operational procedure (SOP), one point lesson (OPL), and check sheet as improvement steps in the production process to maintain product quality.
       
      Laporan tugas akhir ini membahas terkait banyaknya defect flashes dan defect dirty pada produk Cap PPM 03 yang terjadi di perusahaan yang terlihat pada data jumlah defect periode bulan Mei – Oktober 2023 serta penulis menganalisis penyebab terjadinya defect pada produk. Tujuan dari penelitian ini untuk mengurangi jumlah defect yang terjadi pada produk diperusahaan sehingga dapat meminimalkan biaya proses produksi sehingga mendapatkan keuntungan maksimal. Dalam penelitian ini, dilakukan analisis masalah menggunakan metode Six Sigma dengan tahapan DMAIC serta melibatkan alat pengendalian seven tools untuk membantu dalam mengidentifikasi masalah kualitas, menganalisis penyebab akar masalah, dan menemukan solusi yang efektif. Berdasarkan hasil penelitian didapatkan nilai sigma terkecil 3,52 dengan rata-rata 3,80. Menganalisis penyebab akar masalah menggunakan diagram fishbone dan didapati penyebab utama dari defect flashes dan defect dirty berasal dari machine. Hasil dari penelitian yang didapatkan lalu diusulkan pembuatan standar operasional prosedur (SOP), one point lesson (OPL), dan check sheet sebagai langkah perbaikan dalam proses produksi untuk menjaga kualitas dari produk.
