Penyakit Alzheimer merupakan salah satu jenis demensia dengan kasus yang paling banyak dilaporkan. Efek samping akibat konsumsi obat komersial mendorong upaya pengembangan alternatif berbasis tanaman herbal yang lebih aman. Beberapa studi telah melaporkan kombinasi antara kayu manis dengan jahe menghasilkan efek sinergis. Kemampuan kombinasi kedua ekstrak tersebut masih dapat ditingkatkan dengan mencampurkan ke dalam madu rambutan sebagai kandidat inhibitor asetilkolinesterase (AChE). Penelitian ini bertujuan menentukan proporsi optimum ekstrak kombinasi jahe merah dan kayu manis, mengkaji profil antioksidan dan aktivitas inhibisi AChE dari formula berbasis madu rambutan terfortifikasi, serta mengidentifikasi profil metabolit setiap komponen bahan formula. Selain itu, penelitian ini juga menganalisis kadar kumarin pada ekstrak kayu manis sebagai parameter keamanan. Metode penelitian ini mencakup optimasi kombinasi ekstrak menggunakan simplex-lattice design. Proporsi kombinasi ekstrak optimum diformulasikan ke dalam madu rambutan sebesar 5% dan 10% lalu dievaluasi profil antioksidan dan inhibisi AChE. Selain itu, setiap komponen bahan formula dilakukan profiling metabolit menggunakan LC-HRMS. Sebagai parameter keamanan, kuantifikasi kadar kumarin pada ekstrak kayu manis dilakukan menggunakan HPLC. Hasil penelitian ini memperoleh ekstrak kombinasi optimum 33% jahe merah dan 67% kayu manis. Proporsi tersebut memberikan kadar total fenolik (TPC) tertinggi serta kontribusi senyawa bioaktif yang kaya dari kedua sumber tanaman. Fortifikasi ekstrak kombinasi 5% (F2-B) menghasilkan aktivitas inhibisi asetilkolinesterase (AChE) tertinggi sebesar 27,78 ± 2,11% dan memberikan peningkatan signifikan terhadap madu rambutan murni (F2-A). Tidak adanya perbedaan nyata antara penambahan ekstrak kombinasi 5% dan 10% menunjukkan fenomena enzim telah jenuh dan perbedaan mekanisme pengujian. Metabolit yang teridentifikasi dari jahe merah menunjukkan adanya senyawa khas jahe seperti 6-shogaol, 6-gingerdiol, dehidro-6-gingerdion, dehidro-8-gingerdion, dan 10-shogaol. Pada kayu manis, senyawa fenolik seperti asam galat, asam protokatekuat, asam 3-hidroksibenzoat, dan asam salisilat turut terdeteksi yang diduga berkontribusi terhadap kapasitas antioksidan dan mampu berinteraksi dengan situs aktif AChE. Temuan flavan-3-ol seperti cianidanol tidak memiliki gugus keto pada C-4 yang cukup lemah berikatan dengan AlCl3 sehingga tidak terdeteksinya flavonoid pada uji total flavonoid (TFC). Madu rambutan terdeteksi didominasi metabolit primer seperti karbohidrat dan asam amino. Sebagai parameter keamanan, kadar kumarin yang terkuantifikasi sebesar 0,35 mg/g ekstrak kayu manis. Simpulan penelitian ini adalah madu rambutan yang dikombinasikan dengan ekstrak kombinasi 5% (F2-B) merupakan formula optimum dengan aktivitas inhibisi AChE yang baik, didukung profil metabolit bioaktif, serta kadar kumarin yang rendah. Alzheimer's disease (AD) is one of the most prevalent forms of dementia worldwide. The adverse effects associated with commercial drugs have encouraged the development of safer plant-based alternatives. Several studies have reported that the combination of cinnamon and ginger exhibits synergistic effects. This combination may be further enhanced by incorporating it into rambutan honey as a candidate acetylcholinesterase (AChE) inhibitor. This study aimed to determine the optimum proportion of red ginger and cinnamon extract combination, evaluate the antioxidant profile and AChE inhibitory activity of fortified rambutan honey formulations, identify the metabolite profiles of each component, and quantify the coumarin content of cinnamon extract as a safety parameter. The study employed a simplex-lattice design to optimize the extract combination. The optimum extract proportion was incorporated into rambutan honey at concentrations of 5% and 10%, followed by evaluation of antioxidant properties and AChE inhibitory activity. Furthermore, metabolite profiling was performed using liquid chromatography-high resolution mass spectrometry (LC-HRMS). As a safety assessment, coumarin content in cinnamon extract was quantified using high-performance liquid chromatography (HPLC). The optimum extract combination consisted of 33% red ginger and 67% cinnamon. This proportion yielded the highest total phenolic content (TPC) while providing a rich contribution of bioactive compounds from both botanical sources. Fortification with 5% extract combination (F2-B) exhibited the highest AChE inhibitory activity (27.78 ± 2.11%) and showed a significant improvement compared with rambutan honey (F2-A). The absence of a significant difference between the 5% and 10% fortification levels may be attributed to enzyme saturation and differences in assay mechanisms. Metabolite profiling of red ginger identified characteristic ginger constituents, including 6-shogaol, 6-gingerdiol, dehydro-6 gingerdione, dehydro-8-gingerdione, and 10-shogaol. In cinnamon, phenolic
compounds such as gallic acid, protocatechuic acid, 3-hydroxybenzoic acid, and salicylic acid were detected and are presumed to contribute to antioxidant capacity as well as interact with the active site of AChE. The detection of the flavan-3-ol cianidanol, which lacks a C-4 keto group and therefore exhibits weak complexation with AlCl3, may explain the absence of detectable flavonoids. Rambutan honey was predominantly characterized by primary metabolites, particularly carbohydrates and amino acids. As a safety parameter, the quantified coumarin content in
cinnamon extract was 0.35 mg/g extract. In conclusion, rambutan honey fortified with 5% extract combination (F2-B) represents the optimum formulation, demonstrating favorable AChE inhibitory activity supported by a beneficial bioactive metabolite profile and a low coumarin content.
