      “Volume Urin Babi (Sus scrofa domesticus) Model Syok Septik Pasca Terapi Resusitasi Cairan dan Norepinefrin

      Abstract
      Syok septik merupakan kondisi kritis yang ditandai oleh vasodilatasi sistemik dan peningkatan permeabilitas kapiler yang dapat menyebabkan penurunan perfusi organ, termasuk ginjal. Gangguan perfusi ginjal ini berdampak pada penurunan laju filtrasi glomerulus dan produksi urin, sehingga volume urin dapat digunakan sebagai indikator sederhana fungsi ginjal pada kondisi sepsis. Penelitian ini bertujuan untuk mendeskripsikan gambaran volume urin pada babi model syok septik setelah pemberian terapi resusitasi cairan, norepinefrin, dan kombinasi keduanya. Delapan ekor babi jantan dibagi ke dalam empat kelompok perlakuan, yaitu kontrol, sepsis dengan resusitasi cairan, sepsis dengan norepinefrin, dan sepsis dengan kombinasi cairan serta norepinefrin. Syok septik diinduksi menggunakan lipopolisakarida Escherichia coli hingga mencapai mean arterial pressure (MAP) <60 mmHg. Resusitasi cairan dilakukan menggunakan ringer laktat, sedangkan norepinefrin dititrasi untuk mempertahankan MAP =65 mmHg. Volume urin diukur dari vesika urinaria setelah terminasi hewan dan dianalisis secara deskriptif. Hasil penelitian menunjukkan adanya perbedaan volume urin antar kelompok perlakuan. Kelompok sepsis dengan resusitasi cairan menunjukkan penurunan volume urin, sedangkan pemberian norepinefrin meningkatkan volume urin mendekati nilai kontrol. Kelompok kombinasi resusitasi cairan dan norepinefrin menunjukkan volume urin tertinggi, yang mengindikasikan perfusi ginjal yang lebih baik pada babi model syok septik.
       
      Septic shock is a critical condition characterized by systemic vasodilation and increased capillary permeability, which can lead to reduced organ perfusion, including the kidneys. This reduction in renal perfusion results in decreased glomerular filtration rate and urine production; therefore, urine volume can serve as a simple indicator of renal function in sepsis. This study aimed to describe urinary volume of septic shock pig models following fluid resuscitation, norepinephrine administration, and their combination. Eight male pigs were divided into four treatment groups: control, sepsis with fluid resuscitation, sepsis with norepinephrine, and sepsis with combined fluid resuscitation and norepinephrine. Septic shock was induced using lipopolysaccharide from Escherichia coli until mean arterial pressure (MAP) reached <60 mmHg. Fluid resuscitation was performed using ringer’s lactate, while norepinephrine was titrated to maintain MAP =65 mmHg. Urine volume was measured from the urinary bladder after animal termination and analyzed descriptively. The results showed differences in urine volume among treatment groups. The sepsis group receiving fluid resuscitation alone showed a decrease in urine volume, whereas norepinephrine administration increased urine volume closer to control values. The combination of fluid resuscitation and norepinephrine resulted in the highest urine volume, indicating better renal perfusion in the pig septic shock model.
