      Toksisitas Akut (Lethal Dose 50) dan Khasiat Infusa Daun CIplukan sebagai Hepatoprotektor

      Abstract
      Hati mendetoksifikasi obat melalui proses metabolik yang melibatkan keseimbangan oksidan dan antioksidan. Konsumsi obat berlebih dapat mengganggu keseimbangan tersebut dan menyebabkan stres oksidatif yang memicu drug-induced liver injury (DILI). Konsumsi antioksidan eksogen seperti Physalis angulata L. (ciplukan) yang diketahui memiliki aktivitas antioksidan dan hepatoproteksi dapat dijadikan solusi untuk menangani kondisi ini. Penelitian ini bertujuan melakukan karakterisasi infusa daun ciplukan dan khasiat hepatoproteksinya pada kasus DILI. Senyawa metabolit infusa diuji secara kualitatif menggunakan uji fitokimia sederhana, sedangkan aktivitas antioksidannya diuji dengan menggunakan senyawa radikal DPPH. Uji toksisitas akut dilaksanakan dengan pengamatan mortalitas dan gejala toksisitas pada mencit yang diberikan dosis tunggal infusa daun ciplukan 0, 1, 5, 10, dan 15 mg/g. Uji efikasi dilakukan pada tikus yang dibagi ke dalam kelompok kontrol, kontrol negatif, kontrol positif, serta kelompok infusa daun ciplukan dosis 0,3 mg/g dan 0,6 mg/g. Parasetamol (0,5 mg/g) digunakan untuk menginduksi DILI. Parameter yang diamati meliputi kadar malondialdehida (MDA), SGOT, SGPT, dan histopatologi hati. Infusa daun ciplukan mengandung flavonoid, alkaloid, dan tanin dengan aktivitas antioksidan sedang (IC50 160,81 µg/ml) serta bersifat praktis tidak toksik (LD50 >15 mg/g). Kelompok Ciplukan 0,3 mg/g menurunkan kadar enzim hati dan mencegah kerusakan hepatoseluler, sedangkan Ciplukan 0,6 mg/g menurunkan kadar enzim hati namun hanya menunjukkan perbaikan ringan struktur hepatoseluler. Dosis 0,3 mg/g berpotensi sebagai hepatoprotektor, sedangkan dosis 0,6 mg/g menunjukkan potensi efek pro-oksidan ringan pada kasus DILI.
       
      The liver detoxifies drugs through metabolic processes involving oxidant-antioxidant balance. Excessive drug consumption can disrupt this balance, leading to oxidative stress and drug-induced liver injury (DILI). Exogenous antioxidants may help alleviate oxidative stress associated with DILI. Physalis angulata L. (ciplukan) is known to possess antioxidant and hepatoprotective properties. This study aimed to analyze the characteristics, acute toxicity, and hepatoprotective activity of ciplukan leaf infusion in DILI. Metabolite compounds and antioxidant activity of the infusion were evaluated using simple phytochemical screening and the DPPH method. Acute toxicity was assessed in mice administered single doses of 0, 1, 5, 10, and 15 mg/g ciplukan leaf infusion by observing mortality and toxicity symptoms. Hepatoprotective efficacy was evaluated in rats divided into five groups: control, negative control, positive control, and two treatment groups receiving 0.3 mg/g and 0.6 mg/g infusion. Paracetamol (0.5 mg/g) was used to induce DILI. Liver damage was assessed through malondialdehyde (MDA), serum liver enzymes, and liver histopathology. Ciplukan leaf infusion contained flavonoids, alkaloids, and tannins with moderate antioxidant activity (IC50 160.81 µg/mL). The infusion was practically non-toxic with an LD50 >15 mg/g. The 0.3 mg/g dose reduced liver enzyme and MDA levels while maintaining near-normal hepatocellular architecture. The 0.6 mg/g dose showed only slight improvement. Ciplukan leaf infusion at 0.3 mg/g demonstrated hepatoprotective activity, whereas 0.6 mg/g may indicate a mild pro-oxidant effect in DILI.
