      Asosiasi Konsentrasi Partikulat dan Faktor Meteorologi dengan Kejadian Diabetes Melitus: Suatu Tinjauan Pustaka Sistematis

      Abstract
      Particulate matter (PM) merupakan salah satu polutan udara ambien yang sangat memengaruhi kesehatan masyarakat. Konsentrasi partikulat sangat dipengaruhi oleh faktor meteorologi melalui proses dispersi, deposisi, dan transformasi atmosferik. Penelitian ini bertujuan menganalisis hubungan konsentrasi partikulat dan faktor meteorologi terhadap kejadian diabetes melitus melalui tinjauan pustaka sistematis. Metode yang digunakan adalah penelusuran literatur pada basis data Scopus dan ProQuest dengan kata kunci yang mencakup PM, faktor meteorologi, dan diabetes, sehingga diperoleh 62 artikel yang memenuhi kriteria. Faktor meteorologi berperan dalam tiga jalur, yaitu sebagai pengontrol konsentrasi partikulat di atmosfer, paparan independen, dan effect modifier. Suhu udara merupakan faktor yang paling dominan, diikuti kelembapan relatif dan musim, yang memodifikasi besarnya paparan partikulat yang terhirup. Paparan partikulat dapat memengaruhi kondisi metabolik tubuh, dan dapat berkaitan dengan diabetes melitus. Berdasar literatur terpilih, paparan PM2.5 berhubungan dengan peningkatan resistensi insulin, glukosa darah, HbA1c, serta risiko rawat inap dan insidensi diabetes. Studi yang melaporkan efek insidensi dan rawat inap diabetes per kenaikan 10 µg/m³ PM2.5 secara konsisten menunjukkan arah asosiasi positif, dengan peningkatan risiko berkisar 2,6–26% untuk insidensi dan 0,5–3,0% untuk rawat inap. Besaran efek pada biomarker glikemik menunjukkan asosiasi positif walaupun dengan nilai yang bervariasi
       
      Particulate matter (PM) is an ambient air pollutant that substantially affects public health. Particulate concentrations are strongly influenced by meteorological factors through dispersion, deposition, and atmospheric transformation processes. This study aims to analyze the relationship between particulate concentrations, meteorological factors, and the occurrence of diabetes mellitus through a systematic literature review. Literature was retrieved from the Scopus and ProQuest databases using keywords covering particulate, meteorological factors, and diabetes, yielding 62 articles that met the inclusion criteria. Meteorological factors operate through three pathways, namely as controls on atmospheric particulate concentrations, as independent exposures, and as effect modifiers. Air temperature is the most dominant factor, followed by relative humidity and season, which modify the magnitude of inhaled particulate exposure. particulate exposure can affect the body's metabolic condition and may be related to diabetes mellitus. Based on the selected literature, PM2.5 exposure is associated with increased insulin resistance, blood glucose, HbA1c, as well as the risk of hospitalization and diabetes incidence. Studies reporting effects on diabetes incidence and hospitalization per 10 µg/m³ increase in PM2.5 consistently showed a positive direction of association, with risk increases ranging from 2.6–26% for incidence and 0.5–3.0% for hospitalization. Effect estimates for glycemic biomarkers showed positive associations, although with varying magnitudes.
