      Abstract
      PT XYZ merupakan perusahaan yang bergerak di bidang industri kemasan plastik. Salah satu permasalahan utama yang dihadapi adalah tingginya tingkat kecacatan produk cover compact powder yang mencapai 52.556 unit selama periode Juli–November 2025. Berdasarkan analisis pareto, jenis defect "Kotor" menjadi penyumbang terbesar, yakni 59,73% dari total cacat. Penelitian ini menggunakan metode Failure Mode and Effects Analysis (FMEA) untuk menganalisis potensi kegagalan berdasarkan akar penyebab yang diidentifikasi menggunakan diagram fishbone. Hasil analisis menunjukkan bahwa defect disebabkan oleh faktor metode kerja yang belum terstandarisasi dan kurangnya kontrol visual bagi operator. Usulan perbaikan meliputi penerapan One Point Lesson (OPL) sebagai panduan visual standar kebersihan dan checklist Monitoring kebersihan harian untuk meminimalisir kontaminasi produk secara sistematis.
       
      PT XYZ is a company engaged in the plastic packaging industry. A major issue faced is the high defect rate of Cover compact powder products, totaling 52,556 units during the July–November 2025 period. Pareto analysis identified "Dirt" (59.73%) as the primary contributors, accounting for 59,73% of total defects. This study applies the FMEA Method to analyze potential failures based on root causes identified using a fishbone diagram. The results indicate that the defects stem from the lack of standardized operational Methods and inadequate visual control for operators. Proposed improvements include the implementation of a One Point Lesson (OPL) as a visual standard for cleanliness and a daily cleaning Checklist monitoring system to minimize product contamination.
