Sistem Dinamik Penyebaran Penyakit COVID-19 dengan Vaksinasi pada Populasi Rentan
Abstract
COVID-19 adalah penyakit yang disebabkan oleh Severe Acute Respiratory
Coronavirus-2 disingkat SAR CoV-2 dan pertama kali ditemukan di kota Wuhan,
Provinsi Hubei, China pada akhir Desember 2019. COVID-19 dinyatakan sebagai
global pandemic dan di Indonesia dinyatakan sebagai jenis penyakit yang
menimbulkan kedaruratan kesehatan masyarakat serta bencana nonalam, yang
tidak hanya menyebabkan kematian tapi juga menimbulkan kerugian ekonomi
yang cukup besar.
Pencegahan penyebaran penyakit ini dapat dilakukan dengan memberikan
vaksinasi. Vaksinasi yang diberikan hanya pada populasi rentan. Secara medis
individu yang diduga terinfeksi tidak boleh divaksin karena dapat memperburuk
kondisi. Sehingga populasi yang diasumsikan bebas dari penyakit adalah populasi
rentan. Selain itu, dapat dilakukan kajian-kajian dalam bidang pemodelan
matematika dengan intervensi vaksinasi pada model akan sangat berperan,
khususnya pada upaya untuk menekan pertumbuhan populasi terinfeksi.
Tujuan dari penelitian ini adalah memodifikasi model penyebaran penyakit
COVID-19 dari model sebelumnya. Model yang telah dimodifikasi ditentukan
titik tetap beserta bilangan reproduksi dasar (ℛ0
). Kemudian dilakukan analisis
kestabilan dan simulasi numerik untuk melihat dinamika sistemnya. Selanjutnya
dilakukan analisis sensitivitas untuk melihat parameter yang pengaruhnya lebih
besar dari parameter-parameter lainnya terhadap nilai bilangan reproduksi dasar
(ℛ0
).
Pada penelitian ini diperoleh model tipe SVEIR dengan lima populasi
manusia yaitu populasi rentan, populasi vaksin, populasi terpapar, populasi
terinfeksi, dan populasi sembuh. Selain itu, ada penambahan parameter laju
pemberian vaksin, laju kesembuhan vaksin, dan laju populasi yang telah sembuh
menjadi rentan kembali atau laju penurunan kekebalan tubuh. Dari hasil analisis
diperoleh pada model tipe SVEIR terdapat dua titik tetap yaitu titik tetap tanpa
penyakit dan titik tetap endemik. Untuk titik tetap tanpa penyakit jika ℛ0 < 1,
bersifat stabil asimtotik dan titik tetap endemik jika ℛ0 > 1, bersifat stabil
asimtotik lokal. Hasil simulasi numerik menunjukkan kesamaan dengan hasil
analitik. Analisis sensitivitas menghasilkan nilai indeks terbesar yaitu parameter
laju kelahiran alami, laju kematian alami, laju transmisi penyakit, dan laju
pemberian vaksin. Kemudian dilanjutkan dengan simulasi numerik untuk laju
pemberian vaksin dan laju kesembuhan vaksin. Diperoleh jika laju pemberian
vaksin ditingkatkan maka populasi rentan terlihat perbedaan penurunan yang
signifikan, juga terjadi peningkatan pada populasi vaksin. Pada populasi terpapar
dan terinfeksi terlihat lambatnya peningkatan jumlah populasi dan pada populasi
sembuh, jumlah populasi akan semakin bertambah. Semakin tinggi laju
kesembuhan vaksin, maka jumlah populasi manusia rentan terlihat perbedaan COVID-19 is a disease caused by Severe Acute Respiratory Coronavirus-2
abbreviated as SAR CoV-2 and was first discovered in the city of Wuhan, Hubei
Province, China at the end of December 2019. COVID-19 was declared a global
pandemic and in Indonesia it was declared a type of disease. This disease cause
public health emergencies and non-natural disasters, which not only cause death
toll but also substantial economic losses.
Spreading prevention of the disease can be done by mass vaccinations to the
susceptible population. Vaccination is prohibited to those individual with
particular medical condition concern to exacerbated effect induced by
vaccination. So, the rest of population whose free from the exceptional medical
conditions considered as vulnerable population and available as vaccination
target. Furthermore, studies can be carried out in the field of mathematics
modeling with vaccination intervention in the model which will play a very
important role, especially in efforts to suppress the growth of the infected
population
The purpose of this study is to modify the COVID-19 disease spread model
from the previous model. The modified model is determined by a fixed point
along with the basic reproduction number (ℛ0). Then, stability analysis and
numerical simulation were carried out to see the dynamics of the system. Next, a
sensitivity analysis was carried out to see which parameters had a greater
influence than the other parameters on the value of the basic reproduction number
(ℛ0).
In this study, the SVEIR type model consisted of five human populations,
namely the susceptible population, the vaccine population, the exposed
population, the infected population, and the recovered population. Furthermore,
there are additional parameters for the rate of vaccine administration, the rate of
vaccine recovery, and the rate of reinfection or immunity declines. From analysis
applied to SVEIR type model there are two fixed points, namely a fixed point
without disease and an endemic fixed point. For the fixed point without disease if
ℛ0 < 1, it is asymptotically stable and the endemic fixed point if ℛ0 > 1, is
locally asymptotically stable. The numerical simulation results show similarities
with the analytical results. Sensitivity analysis yielded the largest index values,
namely the parameters of natural birth rate, natural death rate, disease
transmission rate, and vaccine administration rate. Then proceed with numerical
simulations for the rate of vaccine administration and vaccine cure rate. It was
found that if the rate of vaccine administration increased, susceptible population
rates decreased significantly different, there was also an increase in the vaccine
population. In the exposed and infected population, the population increases
slowly and in the recovered population, the population will increase. The higher
the vaccine cure rate, the more vulnerable the human population is seen, but the
difference is not significant, in the vaccine population there is an increase in the
population. In exposed and infected populations, the disease persists in the long
term. The recovered human population appears to be stable, in other words, the
population is increasing but the difference in increase is not significant.
