Evaluasi Toksisitas Deksametason secara In Vitro pada Embrio Mencit melalui Analisis Imunofluoresens Mekanisme Hippo Signaling

Deksametason merupakan glukokortikoid sintetik yang banyak diresepkan pada hewan dan manusia. Deksametason merupakan antiemetikum poten yang dilarang diberikan kepada pasien pada fase gestasi karena berbahaya bagi janin dan fetus. Perkembangan keilmuan terakhir menunjukkan adanya indikasi manipulasi deksametason pada mekanisme Hippo signaling (faktor penentu apakah sel embrio menjadi sel trophectoderm/TE sebagai bakal plasenta atau inner cell mass/ICM sebagai bakal fetus). Penelitian ini bertujuan untuk mengevaluasi keamanan deksametason secara in vitro terhadap perkembangan embrio mulai dari 2-sel dan mendapatkan blastosis untuk bahan evaluasi deksametason dalam mengarahkan diferensiasi ICM/TE tersebut. Harapannya, deksametason mampu menginiasi nuklearisasi Yes-associated protein (YAP) ke dalam sel yang diikuti dengan pengarahan diferensiasi sel embrio totipoten menjadi sel TE. Pengujian dilakukan pada Juli 2019 sampai Februari 2021 dengan diawali dengan 2 tahap uji pendahuluan.
Uji pendahuluan 1 adalah optimasi metode pewarnaan imunofluoresens Frum dan Ralston tahun 2019 untuk deteksi YAP, pYAP, dan Cdx2. Pemanjangan waktu fiksasi menjadi 20 menit, waktu permeabilisasi 30 menit, penggunaan konsentrasi anti-YAP dan anti-pYAP 1:100, serta anti-Cdx2 1:200 cukup untuk menghasilkan pendaran yang optimal dan layak digunakan untuk menghasilkan gambar multicolor immunofluorescence staining.
Uji pendahuluan 2 adalah evaluasi dinamika YAP, pYAP, dan Cdx2 pada fase fransisi embrio praimplantasi. Uji ini ditujukan untuk menggambarkan dinamika normal ketiga protein tersebut dalam proses penentuan takdir pertama sel embrio. Sampel penelitian ini mencangkup embrio pada fase transisi (8-sel, morula, dan blastosis). Hasil pengujian menunjukkan bahwa YAP diekspresikan hanya di area nukleus (disebut sebagai sel polar), baik di nukleus dan sitoplasma (sel apolar), atau di sitoplasma saja (sel non-polar). Seiring perkembangan fase transisi, sel polar menurun persentase jumlahnya dan menempati area tepi blastosis. Sebaliknya, sel non-polar semakin meningkat dan menempati area dalam embrio. Sel apolar sebagai transisi antara sel polar dan non-polar ditemukan dalam jumlah sedikit. Di antara ketiga sel tersebut, hanya sel polar yang diekspresikan pada rentang pendaran yang paling luas (dari pendaran rendah/<1.500 piksel hingga pendaran tinggi/>3.000) dan dapat dikelompokkan menjadi empat berdasarkan tingkat polaritasnya, yaitu sel dengan polaritas sangat kuat, kuat, sedang, dan lemah. Protein YAP yang sedang terdegradasi melalui reaksi fosforilasi pada Ser127 (pYAP) sedikit diekspresikan dan spesifik ditemukan di sitoplasma sel. Protein pYAP ditemukan berkorelasi negatif dengan nuklearisasi YAP, yang berarti mobilisasi YAP ke dalam nukleus menurunkan ekspresi pYAP sitoplasma. Sebaliknya, ekspresi Cdx2 didukung oleh nuklearisasi YAP (korelasi positif).
PENELITIAN UTAMA: Evaluasi Toksisitas dan Pengaruh Deksametason pada Mekanisme Hippo Signaling Embrio Praimplantasi. Evaluasi potensi toksik deksametason dilakukan dengan memapar embrio fase 2-sel pada dosis 0, 1, 10, dan 100 µM selama 66 jam. Observasi dilakukan pada kemampuan perkembangan, kapasitas penghambatan nilai cell block, angka blastocyst rate, viabilitas blastosis, rasio sel ICM/TE blastosis, dan ekspresi protein YAP, pYAP, dan Cdx2 sebagai penanda mekanisme Hippo signaling. Hasil yang diperoleh dinarasikan secara deskriptif dan dianalisis dengan ANOVA yang dilanjutkan dengan uji Turkey. Hasil pengujian menunjukkan bahwa deksametason 1 µM ditemukan memiliki 72% blastocyst rate yang setimbang dengan kontrol. Paparan deksametason pada dosis 10 dan 100 µM menghasilkan lebih sedikit blastosis akibat peningkatan angka cell block. Blastosis yang terbentuk seiring dengan paparan deksametason berbagai dosis tersebut memiliki diamater yang nyata lebih lebar daripada perlakuan kontrol, viabilitas sel yang baik (>92,85% adalah sel hidup), dan rasio TE per blastosis yang cukup (<74% TE dan >26% ICM). Oleh karena itu, blastosis tersebut diduga masih memiliki daya dukung yang baik untuk melanjutkan perkembangan selanjutnya.
Terhadap mekanisme Hippo signaling, paparan deksametason hingga dosis 100 µM ditemukan secara tidak signifikan meningkatan total YAP dan total pYAP. Hal ini menandakan bahwa toksisitas deksametason pada embrio tidak melalui mekanisme Hippo signaling. Deksametason mendukung sintesis dan degradasi YAP untuk berjalan secara normal. Besar dugaan bahwa deksametason hanya mempengaruhi sel bagian tepi embrio. Deksametason berbagai dosis tersebut berhasil menginduksi peningkatan jumlah sel polar dan penebalan ekspresi YAP di nukleus sel-sel tepi embrio. Kerapatan sel bagian tepi embrio mencegah keterpaparan sel bagian dalam embrio oleh deksametason. Didukung dengan tight junction dalam jumlah yang banyak, sel-sel bagian dalam embrio tetap berkembang menjadi ICM meski diberi perlakuan deksametason.
Hal yang menarik adalah deksametason dosis 1 µM efektif meningkatkan jumlah sel polar dalam embrio (mampu menginduksi nuklearisasi YAP), mempertahankan polaritas sel polar embrio selama fase transisi, dan memiliki potensi perkembangan yang masih baik. Oleh karena itu deksametason 1 µM disarankan untuk dikaji lebih lanjut untuk menjadi sebagai sediaan pengarah diferensiasi sel menjadi sel TE untuk mendukung rekonstruksi embrio. Dexamethasone is a synthetic glucocorticoid that is prescribed widely in animals and humans. Dexamethasone is a potent anti-emetic but is prohibited from being used on pregnant patients due to its harmful effects on the embryo and fetal development. In recent scientific instigation, dexamethasone is suspected to be able to influence the Hippo signalling mechanism (a lead factor in whether the embryonic cells develop into trophectoderm/TE cells as the pioneer of the placenta or inner cell mass/ICM cells as the origin of the fetal). This study aimed to evaluate the safety of in vitro exposure of dexamethasone to embryonic development starting from 2-cells and obtain blastocysts for evaluation of dexamethasone in directing the differentiation of ICM/TE. Dexamethasone is expected to be able to initiate the nuclearization of Yes-associated protein (YAP) into cells and induce differentiation of totipotent embryonic cells into TE cells. This research was carried out from July 2019 to February 2021, starting with two preliminary studies.
The first preliminary study is the 2019 Frum and Ralston immunofluorescence staining method optimization for YAP, pYAP, and Cdx2 detection. The prolongation of fixation time to 20 minutes, permeabilization time to 30 minutes, and utilization of anti-YAP and anti-pYAP concentrations of 1:100 and anti-Cdx2 1:200 were sufficient to produce optimal luminescence and were suitable multicolour immunofluorescence staining images production.
The second preliminary study is the evaluation of the dynamics of YAP, pYAP, and Cdx2 in the preimplantation embryo transition phase. This study is intended to describe the normal dynamics of these three proteins in the first fate determination process of the embryonic cells. The sample of this study included embryos in the transitional phase (8-cell, morula, and blastocyst). The test results showed that YAP was expressed only in the nuclear area (referred to as polar cells), either in the nucleus and cytoplasm (apolar cells) or in the cytoplasm only (non-polar cells). As the transition phase progresses, polar cells decrease in percent number and occupy the periphery of the blastocyst. Meanwhile, the non-polar cells continue to proliferate and occupy the inside embryo. Apolar cells are the intermediary of polar and non-polar cells and are found in small numbers. Among the three cells, only the polar cells have a broad range of intensity (from low to high, <1.500 until >3.000) and can be grouped into most intense, intense, medium, and mild cells based on their level of polarity. The degraded YAP protein by phosphorylation process on the Ser127 (pYAP) is expressed weakly and solely displayed in the cytoplasm. The pYAP protein discovered has a negative correlation with YAP nuclearization, which means that mobilization of YAP into the nucleus decreased cytoplasmic pYAP expression. In contrast, the intensity of the Cdx2 gene is encouraged by YAP nuclearization (positive correlation).
MAIN RESEARCH STUDY: Evaluation of the Toxicity and Effect Dexamethasone on the Hippo Signaling Mechanism of Preimplantation Embryos. This last phase of the study was conducted by exposing the 2-cell phase embryo at doses of 0, 1, 10, and 100 µM dexamethasone for 66 hours. The observation consisted of the developmental ability, cell block inhibitory capacity, blastocyst rate, blastocyst viability, blastocyst ICM/TE cell ratio, and the intensity of YAP, pYAP, and Cdx2 on behalf of the Hippo signalling mechanism. The data were narrated descriptively and analysed using ANOVA followed by the Turkey test. The test results showed that 1 µM dexamethasone was found to have a 72% blastocyst rate that was comparable to controls. However, dexamethason at a dose of 10 or 100 µM produced a smaller number of blastocysts due to the escalation of the cell block. The exposed-dexamethasone-at-various-doses of blastocyst had a significantly wider diameter than the control treatment, excellent viability (>92,85% were live cells), and a good enough ratio of TE per blastocyst (<74% TE and >26% ICM). Therefore, we suspect that this developed blastocyst still has a good carrying capacity for further development.
Towards the Hippo signalling, dexamethasone up to 100 µM insignificantly increases the total YAP and pYAP. This finding indicates that dexamethasone toxicity in the embryos did not affect the Hippo signalling mechanism. Dexamethasone supports YAP synthesis and degradation to function normally. It is presumed that dexamethasone only affects the peripheral cells of the embryo. These various doses of dexamethasone succeeded in inducing an increase in polar cells number and thickening of nuclear YAP expression in those cells. The density of the outer cells embryo prevents the underneath cells exposed to dexamethasone. Supported by rich tight junctions, the inner cells develop into the ICM even with dexamethasone treatments.
Interestingly, dexamethasone at a dose of 1 µM was effective in increasing the total of polar cells in the embryo (able to induce YAP nuclearization), maintaining the polarity of the embryonic polar cells during the embryonic transition process, and having enough developmental potential. Dexamethasone at a dose of 1 µM is encouraged to be further explored as a TE cell directing differentiation substance to support embryonic reconstruction.
