| Abstract: | Cacat bolong pada kemasan merupakan bentuk pemborosan yang
berdampak pada penurunan kualitas produk dan efisiensi proses produksi.
Penelitian ini bertujuan untuk menganalisis penyebab cacat bolong pada kemasan
mi instan dari Perusahaan PT XYZ serta mengusulkan tindakan pengendalian
dengan pendekatan Six Sigma. Metode Define, Measure, Analyze, Improve, dan
Control (DMAIC) digunakan sebagai kerangka kerja dalam menganalisis data.
Hasil perhitungan performansi sigma menunjukkan nilai sebesar 4,08 pada bulan
September dan 4,09 pada bulan Oktober, dengan Defect Per Million Opportunities
(DPMO) masing-masing sebesar 4.957 dan 4.831. Berdasarkan diagram kendali U
chart, menunjukkan proses produksi belum stabil akibat beberapa titik di luar batas
kendali. Ketidakstabilan ini diperkuat melalui analisis diagram pareto yang
mengidentifikasi tiga jenis cacat utama meliputi sobek/rusak end seal (40,49%),
tertusuk mi (34,61%), dan gencet mi (21,39%). Analisis diagram fishbone
menunjukkan bahwa kerusakan sobek/rusak end seal paling banyak berasal dari
faktor man dan machine; tertusuk mi dari faktor machine dan material; serta gencet
mi dari faktor machine dan method. Verifikasi dilakukan di lapangan untuk
memastikan kondisi aktual dan dijadikan dasar dalam penyusunan tabel kaizen five
m checklist. Pengendalian difokuskan pada pencegahan cacat berulang melalui
penetapan tindakan terhadap faktor penyebab dominan. Hasil penelitian ini dapat
dijadikan acuan untuk peningkatan kualitas dan efisiensi produksi secara
berkelanjutan.

Hole defects in packaging represent a form of waste that contributes to reduced product quality and production efficiency. This study aims to analyze the causes of hole defects in instant noodle packaging produced by Company XYZ and to propose control actions using the Six Sigma approach. The Define, Measure, Analyze, Improve, and Control (DMAIC) method was employed as the analytical framework. Sigma performance calculation yielded values of 4.08 in September and 4.09 in October, with corresponding Defects Per Million Opportunities (DPMO) of 4,957 and 4,831. The U-chart control diagram indicated that the production process was not yet fully stable, as evidenced by several points falling outside the control limits. This instability was further supported by a Pareto analysis identifying three dominant defect types: torn/damaged end seal (40.49%), punctured by noodles (34.61%), and crushed noodles (21.39%). Fishbone diagram analysis indicated that Torn/Damaged End Seal was mostly caused by Man and Machine factors; Noodle Piercing by Machine and Material factors; and Noodle Crushing by Machine and Method factors. Field verification was conducted to confirm actual conditions and served as the basis for developing kaizen five-m checklist table. Control efforts focused on preventing defect recurrence by implementing targeted actions on the dominant causal factors.
