Halmahera Northern districts is an area that has the natural marine resources which contact of iodine is high enough. Fishery Stock of North Halmahera is 89.865 tons/year. But in reality the results of national survey of mumps since 1980 until 2003, North Halmahera including areas class field as severe endemic Iodine Deficiency Disorder (IDD). IDD is one cause of decreased quality of human resources (HR) sector in a region. Therefore, the purpose of this study include: 1) Identification the conditions and diet of fishermen families; 2) Analyze the status (level endemism) of IDD in a family of fishermen in the district North Halmahera.; and 3) To determine the factors (diet) most likely to be the cause of IDD in the district North Halmahera. This research was conducted in 2009 in 10 coastal district in North Halmahera. Identification of goiter or IDD conducted by Palpation/Tactile on the neck and measurement of iodine levels in urine of respondents who were purposively select (purposive sampling) 384 fishing families. Furthermore, respondents were asked to complete a questionnaire or be interviewed about the identify and level of knowledge and diet. Based on interviews or questionnaires and the information that majority of the respondents consume foods containing iodine is high (>100 μg/day) and goitrogenic substance consumption level is high (>10mg/day), while the results of urinary iodine is obtained 0,5% higher, 84,4% of normal and 15,1% lower. Results palpability thyroid gland is known that the total goiter rate (TGR) of 11,7% at district level who fall into categories of mild endemic TGR with reference to the criteria of WHO. This value is lower than the National Goiter Survey in 1980/1982 and the results of the survey year in which the cluster 1995/1996 North Halmahera island of the West with 54,7% TGR. At district level, District Central Tobelo a district that most people with high levels of endemicity of IDD (TGR = 30%) while the lowest Kao District (TGR = 3,3%). Results Logistic regression causing or likely to affect the IDD in fisher family of North Halmahera. Consumption of iodized salt a chance to reduce or prevent IDD or goiter by 0,027. While the consumption if foods containing goitrogenic substance likely to cause or increase the IDD or goiter of 0,101.en
