Profil Bakteri Usus Penderita Hipertensi Berbasis Analisis Metagenomik
Abstract
Hipertensi merupakan salah satu penyakit tidak menular yang menjadi masalah kesehatan sangat serius saat ini, disebut sebagai the silent killer karena kerap terjadi tanpa adanya keluhan. Penyakit hipertensi dipengaruhi oleh faktor genetik, fisiologi dan lingkungan. Perubahan komposisi keragaman mikrobiota di usus dapat menyebabkan ketidakseimbangan mikrobiota yang dapat memicu terjadinya beberapa penyakit diantaranya hipertensi. Kendala yang terjadi saat ini adalah belum terdapat data yang jelas terkait kelimpahan mikrobiota pada penderita hipertensi, sebab kelimpahan mikrobiota pada setiap orang berbeda tergantung jenis asupan makanan yang dikonsumsi. Analisis metagenomik mikrobiota pada penderita hipertensi belum pernah dilaporkan di Indonesia, sehingga kajian mengenai keragaman mikrobiota tersebut perlu dilakukan untuk mengetahui profil dan karakteristik bakteri usus pada penderita hipertensi dan perbandingannya dengan yang tidak menderita hipertensi guna pengembangan pengendalian penyakit hipertensi berbasis mikrobiota.
Penelitian ini diawali dengan pencarian responden untuk dijadikan sampel. Responden diberi kuesioner untuk mendapatkan informasi terkait beberapa karakteristik yang dijadikan sebagai syarat dalam menentukan sampel feses yang akan dianalisis. Kriteria responden yang dapat digunakan adalah sehat, tidak mengkonsumsi antibiotik selama 6 bulan dan tidak mengalami diare. Genom mikrobiota usus diisolasi secara langsung dari sampel feses penderita hipertensi dan non-hipertensi menggunakan Quick-DNA™ Fecal/Soil Microbe Miniprep Kit (USA). Gen 16S rRNA selanjutnya diamplifikasi dari genom bakteri menggunakan primer 516F berlabel fluoresens (6-FAM) dan 1510R. Amplikon gen 16S rRNA dipurifikasi dengan Gel/PCR DNA Fragments Kit (Geneaid Biotech Ltd.) dan hasil purifikasi dipotong dengan enzim restriksi BslI (New England, BioLab). Fragmen hasil pemotongan dianalisis dengan Applied Biosystems Genetic Analyzer dan diinterpretasi menggunakan software analisis GeneMapper® v4.0. Pendugaan untuk setiap unit klasifikasi dan OTU yang sesuai diidentifikasi menurut referensi profiling T-RFLP mikrobiota usus manusia oleh kelompok peneliti Koji Nagashima.
Hasil analisis T-RFLP dengan pemotongan oleh enzim BslI pada 8 sampel feses penderita hipertensi menghasilkan lebih banyak TRF dibandingkan 6 sampel feses non-hipertensi. Analisis Indeks Shannon-Wienner (H’) digunakan untuk mengestimasi keragaman mikrob pada setiap sampel. Indeks keragaman (H’) berdasarkan gen 16S rRNA pada 8 sampel penderita hipertensi berkisar antara 1.00-2.09, sedangkan pada 6 sampel non-hipertensi berkisar antara 0.91-1.62. Indeks kemerataan (E’) gen 16S rRNA berkisar antara 0.80-0.91. Hasil analisis afiliasi dari 26 TRF yang diperoleh terdapat 6 kelompok bakteri pada penderita hipertensi dan non-hipertensi. Pemotongan gen 16S rRNA dengan enzim restriksi BslI berhasil mendeteksi kelompok mikrobiota usus yaitu Clostridium subcluster XIVa, Prevotella, Clostridium cluster IV, Clostridium cluster XI, Bacteroides dan kelompok bakteri lainnya. Perbandingan kelimpahan mikrobiota pada penderita hipertensi lebih tinggi dibandingkan non-hipertensi terlihat pada kelompok Clostridium cluster XI, Clostridium cluster IV dan Clostridium subcluster XIVa. Pada kelompok Bacteroides, penderita hipertensi lebih rendah (42%) dibandingkan non-hipertensi (58%). Persentase rendah pada penderita hipertensi juga ditemukan pada kelompok Prevotella yaitu 8%. Keragaman mikrobiota usus juga dikelompokkan menjadi tiga enterotipe yaitu enterotipe I, enterotipe II dan enterotipe III. Penderita hipertensi memiliki lebih banyak enterotipe III, sedangkan pada non-hipertensi persentase lebih tinggi ada pada Enterotipe I dan Enterotipe II. Rasio F/B pada penelitian ini terjadi peningkatan pada penderita hipertensi. Hypertension is a severe public health problem due to its high prevalence all around the globe. Hypertension is called a "silent killer" because it may have no warning signs or symptoms. The hypertensive disease is influenced by genetic factors, physiology, and environmental factors. Changes in the composition of microbiota diversity in the intestine can cause microbiota imbalances that can trigger several diseases, including hypertension. The current obstacle is that there is no precise data related to the abundance of microbiota in hypertensive patients because the abundance of microbiota in each person is different based on their food. Metagenomic analysis of microbiota in hypertensive patients has never been reported in Indonesia. So studies on microbiota diversity need to be carried out to determine the profile and characteristics of the gut microbiota in hypertension patients and non-hypertension for the development of microbiota-based hypertension disease control.
This study began with the search for respondents to be used as samples. Questionnaires were given to respondents to obtain information related to several characteristics used to determine the faecal sample to be analyzed. Respondent criteria for use are healthy, not taking antibiotics for 6 months, and not having diarrhea.The gut microbiota genome is isolated directly from faecal using the Quick-DNA™ Fecal/Soil Microbe Miniprep Kit (USA). The 16S rRNA gene is further amplified from the bacterial genome using a 516F primer labeled fluorescent (6-FAM) and 1510R. The amplicon of the 16S rRNA gene was calcified with the Gel/PCR DNA Fragments Kit (Geneaid Biotech Ltd.), and was cut with the BslI (New England, BioLab) restriction enzyme. The cut fragments were analyzed with the Applied Biosystems Genetic Analyzer and interpreted using genemapper® analysis software v4.0. Each classification unit and OTU were identified according to the T-RFLP profiling reference of the human gut microbiota based on Koji Nagashima.
The analysis of T-RFLP cutting with the enzyme BslI in 8 faecal samples of hypertension sufferers produced more TRF than 6 samples of non-hypertensive faecal. Analysis of the Shannon-Wienner index (H') was used to estimate the microbial diversity in each sample. The diversity index (H') based on the 16S rRNA gene in 8 samples ranged from 1.00-2.09, while in the 6 samples of non-hypertension, it ranged from 0.91-1.62. The evenness index (E') of the 16S rRNA gene is between 0.80-0.91. The results of the affiliation analysis of the entire TRF cut with the BslI enzyme contained 6 groups of bacteria from 26 TRF in people with hypertension and non-hypertension. The cutting of the 16S rRNA gene with the BslI restriction enzyme successfully detected intestinal microbiota groups, namely Clostridium subcluster XIVa, Prevotella, Clostridium cluster IV, Clostridium cluster XI, Bacteroides, and others. The comparison of microbiota abundance in hypertension patients was higher than non-hypertension seen in Clostridium cluster XI, Clostridium cluster IV, and Clostridium subcluster XIVa. In the Bacteroides group, people with hypertension were lower (42%) than non-hypertension (58%). A low percentage of people with hypertension is also found in the Prevotella group, which is 8%. The diversity of the gut microbiota is also grouped into three enterotypes: enterotype I, enterotype II and enterotype III. Hypertension patients have more enterotype III. While in non-hypertension, a higher percentage is in Enterotype I and Enterotype II. The F / B ratio in this study increased in people with hypertension.
