| Abstract: | PT XYZ merupakan distributor alat berat yang memberikan after-sales service seperti commissioning, periodical service, dan warranty claim. Pada tahun 2024, perusahaan menghadapi tingginya failure rate 17,50% pada unit big vessel. Penelitian ini dilakukan pada Januari hingga Agustus 2025 di PT XYZ dengan tujuan mengidentifikasi jenis failure, mengukur tingkat failure, dan menganalisis faktor-faktor penyebab failure. Metode yang digunakan adalah Six Sigma dengan tahapan Define, Measure, Analyze, menggunakan alat bantu CTQ, pareto chart, DPMO, diagram sebab-akibat, dan FMEA. Hasil CTQ menunjukkan empat jenis failure, dengan structural dan dimensional sebagai failure yang paling dominan. Nilai DPMO sebesar 43.750 dan level sigma 3,20 menunjukkan bahwa kualitas proses masih di bawah standar Six Sigma. Berdasarkan pareto chart, 80% failure disebabkan oleh dua kategori tersebut. Faktor penyebab berasal dari aspek man, machine, method, dan material. FMEA menunjukkan nilai RPN tertinggi pada ketidaksesuaian SOP 274,32 dan misalignment komponen 286,81. Temuan ini menjadi dasar bagi perusahaan untuk melakukan perbaikan terhadap mutu unit big vessel. PT XYZ is a heavy equipment distributor that provides after-sales services such as commissioning, periodical service, and warranty claims. In 2024, the company faced a high failure rate of 17.50% on its big vessel units. This study was conducted from January to August 2025 at PT XYZ with the aim of identifying the types of failures, measuring the failure rate, and analyzing the factors causing the failures. The method used was Six Sigma with the Define, Measure, Analyze stages, using CTQ, pareto chart, DPMO, cause-and-effect diagram, and FMEA tools. The CTQ results showed four types of failures, with structural and dimensional being the most dominant. The DPMO value of 43.750 and sigma level of 3,20 indicated that the process quality was still below the Six Sigma standard. Based on the Pareto chart, 80% of failures were caused by these two categories. The root causes originated from the aspects of man, machine, method, and material. FMEA showed the highest RPN values for SOP non-compliance at 274,32 and component misalignment at 286,81. These findings serve as the basis for the company to implement improvements in the quality of big vessel units. |
