| Abstract: | PT Natamas Plast merupakan perusahaan yang bergerak di bidang industri kemasan plastik. Salah satu permasalahan utama yang dihadapi adalah tingginya tingkat kecacatan produk yang melebihi batas toleransi perusahaan. Berdasarkan data produksi, Botol XYZ memiliki tingkat kecacatan tertinggi, dengan delapan jenis cacat yang teridentifikasi. Dari seluruh cacat tersebut, miss printing (49%) dan kotor (31%) menjadi penyumbang terbesar, yakni 80% dari total cacat. Penelitian ini menggunakan metode FMEA untuk menganalisis potensi kegagalan berdasarkan akar penyebab yang telah diidentifikasi menggunakan diagram fishbone. Hasil analisis menunjukkan bahwa cacat disebabkan oleh faktor mesin, metode kerja, dan lingkungan. Usulan perbaikan meliputi penyediaan wadah khusus untuk mixing tinta, penguatan prosedur pengadukan, pemasangan sensor dan PLC, penerapan formulir FAI, serta penguatan prosedur kebersihan mesin dan bahan baku. PT Natamas Plast is a company engaged in the plastic packaging industry. A major issue faced is the high defect rate exceeding the company's tolerance limits. Based on production data, XYZ bottles show the highest defect rate, with eight types of defects identified. Among these, miss printing (49%) and contamination (31%) account for 80% of total defects. This study applies the FMEA method to analyze potential failures based on root causes identified using a fishbone diagram. The results indicate that defects stem from machine instability, improper work methods, and poor environmental control. Proposed improvements include providing dedicated containers for ink mixing, enhancing mixing procedures, installing sensors and PLCs, implementing FAI forms, and reinforcing cleaning and raw material handling standards. |
