Abstract: Sistem Manajemen Keselamatan dan Kesehatan Kerja (SMK3) merupakan suatu program yang wajib dilaksanakan oleh setiap perusahaan. Hazard Identification Risk Assessment and Determining Controls (HIRADC) merupakan metode yang digunakan untuk mengidentifikasi potensi bahaya dan risiko, potensi bahaya yang menjadi sorotan yaitu bahaya slip, trip and fall. Tujuan dari penelitian yaitu mengidentifikasi faktor penyebab, menghitung level bahaya dan risiko, membuat upaya pengendalian bahaya dan risiko slip, trip and fall serta merancang sistem pelaporan kecelakaan kerja berbasis website. Data primer yang digunakan bersumber dari hasil observasi dan wawancara, sedangkan data sekunder bersumber dari dokumen HIRADC perusahaan dan studi literatur. Analisis yang dilakukan menggunakan metode HIRADC. Bahaya slip, trip and fall memiliki delapan faktor penyebab. Menghitung level risiko menggunakan analisis nilai likelihood dan severity. Pengendalian dilakukan sesuai dengan hierarki K3, selain itu diperlukan sebuah sistem pelaporan kecelakaan kerja berbasis website yang untuk melaporkan segala kejadian (unsafe action, unsafe condition, nearmiss). Sistem pelaporan yang dirancang menggunakan metode agile karena sifatnya yang fleksibel. Tahapan metode agile yaitu, perencanaan, implementasi, tes perangkat lunak, dokumentasi, deployment, dan pemeliharaan. Implementasi sistem dilakukan menggunakan safety sign yang berisi barcode mengenai tata cara pelaporan yang ditempel di setiap area kerja.
Occupational Safety and Health Management System (SMK3) is a program that must be implemented by every company. Hazard Identification Risk Assessment and Determining Controls (HIRADC) is a method used to identify potential hazards and risks, the potential hazards that are highlighted are slip, trip and fall hazards. The purpose of the study is to identify causal factors, calculate the level of hazard and risk, make efforts to control slip, trip and fall hazards and risks and design a website-based work accident reporting system. The primary data used comes from the results of observations and interviews, while secondary data comes from the company's HIRADC documents and literature studies. The analysis used the HIRADC method. Slip, trip and fall hazards have eight causal factors. Calculating the risk level using likelihood and severity value analysis. Control is carried out in accordance with the K3 hierarchy, in addition a website-based work accident reporting system is needed to report all incidents (unsafe action, unsafe condition, nearmiss). The reporting system designed uses the agile method because of its flexible nature. The stages of the agile method are planning, implementation, software testing, documentation, deployment, and maintenance. The system implementation is carried out using safety signs containing barcodes regarding reporting procedures which are attached to each work area.
