Abstract: | Rabies merupakan penyakit virus yang menyerang sistem saraf pusat
mamalia termasuk manusia. Rabies menjadi salah satu zoonosis prioritas di
Indonesia dan telah menyebar di 26 dari 34 provinsi di Indonesia. Kalimantan Barat
merupakan salah satu provinsi di Indonesia yang berstatus tertular rabies. Wilayah
yang luas, sumberdaya manusia, anggaran dan fasilitas pelayanan kesehatan hewan
yang terbatas menjadi tantangan bagi Pemerintah Provinsi Kalimantan Barat dalam
pencegahan dan pengendalian rabies sehingga diperlukan adanya strategi yang
efektif. Pemerintah Provinsi Kalimantan Barat telah melakukan upaya pencegahan
dan pengendalian rabies saat sebelum intervensi dengan pendekatan One Health
(tahun 2014 hingga tahun 2016) dan setelah intervensi dengan pendekatan One
Health (tahun 2017 hingga tahun 2020). Kabupaten Ketapang merupakan wilayah
percontohan penerapan One Health di Kalimantan Barat.
Penyakit ini memberikan dampak ekonomi dan sosial. Setiap kejadian
penyakit selalu menimbulkan beban bagi komunitas atau negara. Pengukuran beban
penyakit menggunakan konsep Global Burden Disease (GBD) yaitu ukuran
sederhana kesehatan populasi. Metrik GBD yang diterima secara luas dikenal
dengan DALY (Disability-Adjusted Life Years). Hal terkait zoonosis digunakan
pengukuran dengan konsep Zoonotic Disability-Adjusted Life Years (zDALY) yaitu
mengkombinasikan jumlah tahun hidup pada kematian dini (Years Life Lost/YLL);
jumlah tahun hidup dengan disabilitas (Years Live with Disability/YLD) serta nilai
lokal hewan dan produknya terkait morbiditas dan mortalitas pada ternak atau
hewan domestik lainnya akibat penyakit (Animal Loss Equivalent/ALE).
Evaluasi ekonomi dan kajian pengukuran nilai DALY dari beban rabies
dibutuhkan oleh pengambil keputusan sehingga dapat memberikan informasi
tambahan pentingnya dilakukan program pengendalian rabies. Penelitian ini
bertujuan untuk menghitung beban penyakit menggunakan zDALY (Zoonoses
Disability Adjusted Life Years) saat sebelum dan setelah adanya intervensi dengan
pendekatan One Health di Kabupaten Ketapang, Kalimantan Barat; serta
menganalisis secara ekonomi program pengendalian rabies yang telah dilakukan
saat sebelum dan setelah intervensi dengan pendekatan One Health.
Data primer dan sekunder digunakan untuk penelitian ini. Data primer
dikumpulkan melalui wawancara dan focus group discussion (FGD), dan data
sekunder diperoleh dari Pemerintah Provinsi Kalimantan Barat dan Pemerintah
Kabupaten Ketapang. Hasil penelitian ini menunjukkan beban penyakit dari rabies
pada sektor kesehatan hewan dan sektor kesehatan manusia sebelum adanya
intervensi dengan pendekatan One Health di Kabupaten Ketapang diperoleh nilai
zDALY sebesar 1.561,22 tahun-orang (109,53 tahun per 100.000 orang),
sedangkan beban penyakit setelah adanya intervensi dengan pendekatan One
Health diperoleh nilai zDALY sebesar 1.808,86 tahun-orang (86,62 tahun per
100.00 penduduk).
Analisis biaya-manfaat (CBA) saat sebelum adanya intervensi dengan
pendekatan One Health memiliki net present Value (NPV) negatif sebesar 583 Juta
Rupiah dan rasio manfaat-biaya (BCR) kurang dari 1 (0,863), sedangkan indikator
CBA saat setelah adanya intervensi dengan pendekatan One Health memiliki nilai
NPV positif sebesar 1,2 Miliar Rupiah dan BCR lebih dari 1 (1,33). Hal ini
menunjukkan rate zDALY dengan pendekatan One Health lebih kecil
dibandingkan dengan tanpa pendekatan One Health. Beban penyakit akibat rabies
akan menjadi lebih tinggi bila tidak dilakukan pencegahan penyebaran penyakit
kepada manusia. Indikator CBA menunjukkan program pengendalian rabies
dengan pendekatan One Health yang telah dilakukan di Kabupaten Ketapang,
Kalimantan Barat lebih menguntungkan secara ekonomi dan layak dijalankan
dibandingkan dengan tanpa pendekatan One Health. Rabies is a viral disease that Rabies is one of the prioritized zoonoses in Indonesia and has spread to 26 out of 34 provinces in Indonesia.attacks the central nervous system of mammals, including humans. West Kalimantan is one of the provinces which has contracted rabies. The large area, human resources, budget, and limited animal health service facilities challenge the Provincial Government of West Kalimantan in preventing and controlling rabies, so an effective strategy is needed. The Provincial Government of West Kalimantan has made efforts to prevent and control rabies before the intervention using the One Health approach (2014 to 2016) and after the intervention using the One Health approach (2017 to 2020). Ketapang District is a pilot area for implementing One Health in West Kalimantan. This disease has an economic and social impact. Disease occurrence always poses a burden to the community or country. The concept of Global Burden Disease (GBD) is used to measure disease burden, a simple measure of population health. The widely accepted GBD metric is DALY (Disability-Adjusted Life Years). Zoonoses measurements use the Zoonotic Disability-Adjusted Life Years (zDALY) concept. The concept of zDALY combines the number of years lived at early death (Years Life Lost/YLL); the number of years lived with disabilities (Years Live with Disability/YLD), as well as local values of animals and their products related to morbidity and mortality in livestock or other domestic animals due to disease (Animal Loss Equivalent/ALE). Economic evaluations and studies on the measurement of the DALY value of the rabies burden are needed by decision makers so that they can provide additional information on the importance of carrying out a rabies control program. This study aims to assess disease burden using zDALY (Zoonoses Disability Adjusted Life Years) before and after intervention with the One Health approach in Ketapang District, West Kalimantan; as well as analyzing the economics of rabies control programs that have been carried out before and after the intervention with the One Health approach. Primary and secondary data are used for this research. Primary data was collected through interviews and focus group discussions (FGD), and secondary data was obtained from the Provincial Government of West Kalimantan and the Government of Ketapang District. The results of this study show that the disease burden on animal health and human health before the intervention with the One Health approach in Ketapang District was zDALY value of 1,561.22 person-years (109.53 years per 100,000 people), while the disease burden after the intervention with the One Health approach obtained by zDALY value of 1,808.86 person-years (86.62 years per 100,000 population). The cost-benefit analysis (CBA) before the intervention with the One Health approach had a negative net present value (NPV) of 583 Million Rupiah. The benefit-cost ratio (BCR) was less than 1 (0.863), while the CBA indicator after the intervention of the One Health approach has a positive NPV value of 1.2 billion Rupiah and a BCR of more than 1 (1.33). The result shows that the zDALY rate with the One Health approach is lower than without the One Health approach. The disease burden due to rabies will be higher if prevention of the spread of the disease to humans is not carried out. The CBA indicator shows that the rabies control program using the One Health approach implemented in Ketapang District, West Kalimantan is more economically profitable and feasible than without using the One Health approach. |
