      Efek Sistemis Cedera Iskemia-Reperfusi dan Terapi Ultrasound terhadap Organ Tikus

      Abstract
      Cedera Iskemia-Reperfusi (I/R) merupakan cedera yang disebabkan oleh reaksi biokimia yang timbul akibat reperfusi setelah periode iskemia. Penelitian ini bertujuan untuk melakukan evaluasi kerusakan histologi berbagai organ, motilitas usus, kadar CKMB, serta LDH setelah cedera I/R dan terapi ultrasound (US) pada nervus vagus. Penelitian dilakukan pada 24 ekor tikus Sprague-Dawley yang dibagi menjadi 4 kelompok yaitu naif (N), kontrol (C), no treatment (NTR), dan treatment (L). Iskemia diinduksi dengan ligasi arteri femoralis selama 6 jam dan dilanjutkan dengan reperfusi selama 30 menit. Terapi US diberikan pada kelompok L dengan intensitas 1,5 W/cm2 . Pengamatan histopatologi organ paru-paru, hati, dan usus halus dari kelompok yang terdampak cedera I/R mengalami perubahan patologis ringan. Tidak terlihat kelainan pada otot jantung, limpa, dan ginjal. Data histopatologi kelompok L mengindikasikan bahwa pemberian terapi tidak memperburuk kerusakan jaringan. Pada kelompok terdampak cedera I/R, motilitas usus halus mengalami penurunan secara signifikan. Kadar CK-MB dan LDH memiliki kecenderungan untuk meningkat yang menandakan adanya kerusakan jaringan. Terapi US selama reperfusi menunjukkan pengembalian motilitas usus halus, serta kadar CK-MB dan LDH mendekati nilai normal. Metode ligasi arteri pada penelitian ini menyebabkan derajat iskemik yang ringan, sehingga cedera I/R yang dihasilkan juga ringan. Terapi US memiliki kencenderungan untuk memperbaiki kerusakan jaringan akibat cedera I/R.
       
      Ischemia reperfusion injury (IRI) is an injury caused by a biochemical reaction arising from reperfusion after a period of ischemia. This study was conducted to evaluate the histological damage of various organs, intestinal motility, CK-MB, and LDH levels after IRI and ultrasound (US) therapy on the vagus nerve. The study was carried out on 24 Sprague-Dawley rats which were divided into 4 groups namely naive (N), control (C), no treatment (NTR), and treatment (L). Ischemia was induced by femoral artery ligation for 6 hours and followed by reperfusion for 30 minutes. Therapy was given to group L with an intensity of 1,5 W/cm2 . Based on histopathological observations, the lungs, liver and small intestine of the groupd affected by IRI experienced mild pathological changes. No abnormalities were found in the heart muscle, spleen, and kidney. The histopathological data of group L indicated that the therapy did not exacerbate tissue damage. In the IRI-affected groups, the intestinal motility were decreased significantly. CK-MB and LDH levels tend to increase which indicates tissue damage. Ultrasound therapy during reperfusion was able to attenuate intestinal motility, CK-MB, and LDH levels close to normal values. The arterial ligation method used resulted in a mild ischemic degree, therefore resulting mild reperfusion injury. Ultrasound therapy has a tendency to repair tissue damage caused by IRI.
