PT X merupakan perusahaan manufaktur peralatan bedah mata yang menghadapi permasalahan tingginya persediaan Work-in-Process (WIP) pada material MxL akibat penerapan push system yang memicu waste berupa inventory. Selain itu, dalam monitoring dan evaluasi WIP masih terdapat waste of waiting akibat adanya aktivitas Business Non-Value-Added (BNVA). Penelitian ini mengaplikasikan metode Root Cause Analysis (RCA) melalui pendekatan 5 Whys, perancangan pull system berbasis kanban dengan pendekatan Just-in-Time, serta otomatisasi aliran informasi dengan Visual Basic for Applications (VBA) ke database dan tools Business Intelligence (BI), yaitu Metabase. Hasil evaluasi komparatif membuktikan bahwa simulasi kanban dengan ukuran lot 4.000 unit per box mampu menurunkan akumulasi WIP dan stock value (IDR) secara signifikan dengan persentase penurunan berkisar antara 56% hingga 79%. Selain itu, replenishment time per box material juga berhasil diakselerasi sebesar 50%, yakni dari 0,23 hari menjadi 0,13 hari. Pada aspek aliran informasi daily report Production yang terintegrasi VBA dan dasbor metabase sukses menghilangkan Business Non-Value Added (BNVA) dari tiga menjadi satu dengan mereduksi waktu siklus pelaporan menjadi real-time setelah penginputan.; PT X is a manufacturer of ophthalmic surgical equipment that faces excessive Work-in-Process (WIP) inventory of MxL material due to the implementation of a push Production system, resulting in inventory waste. In addition, the monitoring and evaluation of WIP are affected by waiting waste caused by Business Non-Value-Added (BNVA) activities. This study applies Root Cause Analysis (RCA) using the 5 Whys approach, designs a kanban-based pull system following the Just-in-Time (JIT) philosophy, and develops an automated information flow system using Visual Basic for Applications (VBA), a database, and the Business Intelligence (BI) platform Metabase. The comparative evaluation results demonstrate that the proposed kanban simulation with a lot size of 4.000 units per box significantly reduces WIP accumulation and inventory value, with reductions ranging from 56% to 79%. Furthermore, the replenishment time per material box is reduced by 50%, from 0,23 days to 0,13 days. From an information flow perspective, the integration of daily Production reporting with VBA and a Metabase dashboard reduces Business Non-Value-Added (BNVA) activities from three to one with enables real-time reporting immediately after data entry.

Penelitian dilaksanakan sebagai langkah utama dalam menganalisis tingkat &#13;
downtime yang disebabkan oleh pengendalian material yang tidak akurat. Tujuan &#13;
dari penelitian ini adalah untuk mengidentifikasi material prioritas penyebab &#13;
downtime serta mengevaluasi tingkat service level yang dihasilkan. Metode yang &#13;
digunakan meliputi analisis ABC untuk menentukan prioritas material, analisis pola &#13;
pergerakan material, serta simulasi untuk mengukur perbaikan sistem usulan. Hasil &#13;
penelitian menunjukkan bahwa material P840JFS CMX 20007 Black dan PP AZ &#13;
564GL 202B Black menjadi penyumbang utama terjadinya downtime, dengan &#13;
tingkat service level aktual yang belum optimal. Setelah dilakukan simulasi, terjadi &#13;
peningkatan service level yang ditandai dengan penurunan frekuensi downtime. &#13;
Dengan demikian, metode yang diusulkan dapat meningkatkan ketersediaan &#13;
material dan meminimalkan potensi downtime pada sistem pengendalian &#13;
persediaan.; This study was conducted as a primary step in analyzing the level of &#13;
downtime caused by inaccurate material inventory control. The objective of this &#13;
research is to identify priority materials contributing to downtime and to evaluate &#13;
the resulting service level. The methods employed include ABC analysis to &#13;
determine material priorities, analysis of material movement patterns, and &#13;
simulation to assess improvements in the proposed system. The results indicate that &#13;
materials P840JFS CMX 20007 Black and PP AZ 564GL 202B Black are the main &#13;
contributors to downtime, with the actual service level remaining suboptimal. &#13;
Following the simulation, an improvement in service level is observed, as indicated &#13;
by a reduction in the frequency of downtime. Therefore, the proposed method is &#13;
capable of enhancing material availability and minimizing the potential for &#13;
downtime within the inventory control system.

Penelitian ini bertujuan untuk mengetahui jenis-jenis defect produk book paper 55 gsm, menganalisis akar penyebab defect, mengetahui tingkat risiko defect, serta merumuskan tindakan perbaikan pada proses produksi book paper 55 gsm menggunakan metode Six Sigma dengan pendekatan DMAIC. Berdasarkan analisis laporan produksi periode Mei–November 2025, broke terbesar berasal dari proses Paper Machine 3 sebesar 33% dengan empat jenis defect yang teridentifikasi, yaitu keriput PM (KP), flui (PL), legok (LG), dan gelombang (GL). Defect flui (PL) ditetapkan sebagai defect dominan berdasarkan diagram pareto. Analisis akar penyebab menggunakan diagram sebab akibat 4M+1E dan CFME menghasilkan delapan cause of failure, dengan penyebab RPN tertinggi berdasarkan FMEA adalah tidak adanya standar perlindungan basepaper (RPN = 217). Tindakan perbaikan diimplementasikan dalam bentuk One Point Lesson (OPL) dan checksheet, menghasilkan penurunan DPMO dari 12.752,80 menjadi 9.390,67 serta peningkatan level sigma dari 3,73 menjadi 3,85 dengan tingkat kepuasan pelanggan sebesar 93,3%.; This study aims to identify types of defects, analyze root causes, determine defect risk levels, and formulate corrective actions in the production process of 55 gsm book paper using the Six Sigma method with a DMAIC approach. Based on the analysis of production reports for the period of May–November 2025, the largest broke originated from the Paper Machine 3 process at 33%, with four types of defects identified, namely wrinkle PM (KP), flui (PL), concave (LG), and wavy (GL). Flui defect (PL) was established as the dominant defect based on the Pareto diagram. Root cause analysis using a fishbone diagram with a 4M+1E approach and CFME identified eight causes of failure, with the highest RPN cause based on FMEA being the absence of basepaper protection standards (RPN = 217). Corrective actions were implemented in the form of One Point Lessons (OPL) and checklists, resulting in a reduction of DPMO from 12,752.80 to 9,390.67 and an improvement in sigma level from 3.73 to 3.85 with a customer satisfaction rate of 93.3%.

Mesin CNC Plasma Cutting memiliki peran penting dalam proses produksi&#13;
fabrikasi logam di PT Panca Wahana Teknindo. Penelitian ini bertujuan untuk&#13;
mengukur efektivitas dan keandalan mesin menggunakan metode Overall&#13;
Equipment Effectiveness (OEE), Mean Time Between Failure (MTBF), Mean Time&#13;
To Repair (MTTR), dan Mean Down Time (MDT). Analisis dilakukan&#13;
menggunakan pendekatan Six Big Losses dan Root Cause Analysis untuk&#13;
mengidentifikasi penyebab kerugian dan kerusakan mesin. Hasil penelitian&#13;
menunjukkan nilai OEE tahunan sebesar 97,16% dengan availability 90,58%,&#13;
performance 109,49%, dan quality yield 97,81%. Kerugian dominan yang&#13;
ditemukan yaitu breakdown losses, reduced speed losses, dan process defect losses&#13;
yang disebabkan oleh belum optimalnya inspeksi harian, pencatan checksheet,&#13;
autonomous maintenance, dan preventive maintenance. Dengan demikian,&#13;
Penerapan Total Productive Maintenance diharapkan dapat meningkatkan&#13;
keandanlan dan efektivitas operasional mesin CNC Plasma Cutting.; The CNC Plasma Cutting machine has an improtant role in metal fabrication&#13;
production process at PT Panca Wahana Teknindo. This study aims to measure the&#13;
effectiveness and reliability of the machine using the Overall Equipment&#13;
Effectiveness (OEE), Mean Time Between Failure (MTBF), Mean Time To Repair&#13;
(MTTR), and Mean Down Time (MDT) methods. The analysis was conduted using&#13;
the Six Big Losses and Root Cause Analysis approaches to identify the causes of&#13;
losses and machine failures. The results showed that the annual OEE value was&#13;
97.16%, with availability of 90.58%, performance of 109.49%, and quality yield of&#13;
97.81%. The dominant losses found were breakdown losses, reduced speed losses,&#13;
and process defect losses caused by suboptimal daily inspections, checksheet&#13;
recording, autonomous maintenance, and preventive maintenance. Therefore, the&#13;
implementation of Total Productive Maintenance is ecpected to improve the&#13;
reliability and operational effectiveness of the CNC Plasma Cutting machine.
