      Studi Kasus: Penerapan Split Thickness Skin Graft pada Kucing Domestic Short Hair

      Abstract
      Split Thickness Skin Graft (STSG) atau cangkok kulit berisi epidermis dan sebagian dermis merupakan salah satu metode untuk mengatasi kerusakan kulit yang luas. Tujuan dari studi kasus ini adalah mengamati dan menganalisis penerapan STSG pada pasien kucing ras domestic short hair,berusia 3,5 tahun dengan kondisi luka terbuka dan kehilangan seluruh kulit pada ekstremitas bawah kanan depan yang cukup parah, sehingga mengalami kesulitan untuk berjalan dengan normal. Sebelum operasi dilakukan, luka diobati dengan Bioplacenton® dan NaCl 0,9% selama 5 hari, serta dilakukan pemeriksaan analisa darah dan X-ray. Gambaran darah pasien yaitu, kadar trombosit (103 /µL), dan MPV (fL) mengalami penurunan, kadar leukosit (103 /µL), limfosit (%), monosit (103 /µL), monosit (%), serta granulosit (%) mengalami peningkatan. Luka kemudian diberi penangan, sehingga menunjukkan infeksi berhasil ditangani. Hasil pemeriksaan X-ray tidak ditemukan indikasi fraktur pada kaki pasien yang terluka. Penanganan pasien diawali dengan pemanenan kulit untuk pencangkokan STSG pada daerah right dorsal M. latissimus dorsi yang berasal dari pasien sendiri. Cangkok kulit kemudian dijahit di area luka menggunakan non-absorbable suture, yaitu polypropylene suture 3/0. Immobilisasi cangkok kulit dilakukan dengan teknik Tie-over Dressing selama 10 hari. Terapi yang diberikan pasca operasi yaitu pemberian analgesik, anantibiotik (oral dan topikal), suplemen, pembalutan luka, perawatan luka, serta pemantauan persembuhan luka. Penutupan luka pasca operasi di daerah donor terjadi pada bulan ke-3. Persembuhan luka di area resipien terjadi secara bertahap sampai hari ke-180 dengan kondisi luka yang menunjukkan pertumbuhan kulit sempurna sebanyak 80% area luka dan granulasi dermis di sekitar 20% area luka. Pertumbuhan kulit baru terlihat menyatu dengan kulit di sekitarnya menunjukkan proses pemulihan yang baik. Berdasarkan studi kasus ini, STSG merupakan salah satu alternatif yang tepat dalam penanganan luka terbuka yang luas.
       
      Split Thickness Skin Graft (STSG) or skin grafting containing the epidermis and part of the dermis is one method to overcome extensive skin damage. The purpose of this case study is to observe and analyze the application of STSG in a 3.5-year-old domestic short-haired cat patient with an open wound and severe loss of all skin on the right front lower extremity, making it difficult for him to walk normally. Before the operation, the wound was treated with Bioplacenton® and 0.9% NaCl for 5 days, and supporting blood analysis and X-ray examinations were performed. The patient's blood picture showed that platelet levels (103 /µL), and MPV (fL) decreased, leukocyte levels (103 /µL), lymphocytes (%), monocytes (103 /µL), monocytes (%), and granulocytes (%) increased. The wound was then treated, indicating that the infection had been successfully treated. The X-ray examination results did not find any indication of fracture in the patient's injured leg. Patient treatment began with skin harvesting for STSG grafting in the right dorsal M. latissimus dorsi area from the patient himself. The skin graft was then sutured in the wound area using a non-absorbable polypropylene suture 3/0. Immobilization of the skin graft was performed using the Tie-over Dressing technique for 10 days. Therapy after surgery includes administering analgesics, antibiotics (oral and topical), supplements, wound dressings, wound care, and monitoring wound healing. Post-operative wound closure in the donor area occurred in the 3rd month. Wound healing in the recipient area occurred gradually until the 180th day with wound conditions showing perfect skin growth in 80% of the wound area and dermis granulation in about 20%. New skin growth was seen to blend with the surrounding skin indicating a good healing process. Based on this case study, STSG is one of the right alternatives for handling large open wounds.
