The implementation of the FT Lomanis MSME mentoring program has obstacles, including weak mentoring and no evaluation of the program. This study aims to evaluate the FT Lomanis MSME mentoring program using the CIPP evaluation model and formulate program improvement initiatives through the business incubator model. The study used a qualitative approach with data collection through observation, interviews and documentation. Data was processed with the stages of data reduction, data display and conclusions and using data validity checks. The results of the study showed that the program requires in-depth identification of needs for its targets (Context), there are limitations in human resources and the abilities of instructors and mentors (Input), the program implementation process is not optimal (Process), and there are several successes in short-term goals and shortcomings in several areas that need to be improved (Product). To overcome these weaknesses, the researcher formulated a business incubator model which was designed into three stages, namely the pre-incubation, incubation, and post-incubation stages.
